This notice has been prepared under Türkiye's Personal Data Protection Law No. 6698 ("KVKK") by Physiotherapist Reşit Taş — RETAFİZ Sağlık ("the Clinic") as data controller. Address: Akat Mah. Hare Sokak No:20, Beşiktaş / İstanbul.

Identity and contact data: full name, phone, e-mail.
Health data: complaint history, assessment findings, treatment records (special-category data; processed only with your explicit consent).
Transaction-security data: IP address and cookie records.

Receiving, scheduling and reminding of appointment requests
Providing physiotherapy assessment and treatment services
Fulfilling legal obligations
